Despite continued advances in medical care and delivery, health disparities still exist across underserved populations in the US. From access to continuity and coordination of care, minority and low-income populations can trail their white and high-income counterparts by up to 20 percent. With the advent of health IT, solutions to overcoming health disparities have begun to emerge. The rise of the internet and proliferation of mobile wireless internet access have given providers and patients new ways to connect and stay connected.
